I suspect it has to do with Mac OS El Capitan.
This happens after upgrading.

try using this to format your SD card. Use the Overwrite function:  https://www.sdcard.org/downloads/formatter_4/
Also, be sure you download the config file to your desktop then drag it to your SD card.
