Stereo dropping to mono

I have two silver phantoms and dialog and recently being dropping from stereo sound to mono momentarily and intermittently and its very annoying.  Anyone else?  Any help or advice welcome!

Have you tried a full system restart? I didn't have the exact issue as you, but when changing input sources, my system would more often than not lose one channel. Restarting Dialog would fix the issue, but only temporarily. I then did a "full" system restart and now I have yet to encounter that again for almost a week now. Prior, I would see the issue daily.
